INsight Makeover
Rules: NO PURCHASE NECESSARY
Entry deadline: noon October 31, 2013
Must be over 18 to enter.
One person will be chosen every week to receive a makeover. Winner will be notified via phone or email. Winner must contact salon to schedule appointment with participating salon. Winner must provide their own transportation and live within our viewing area. Makeover must be scheduled prior to December 31, 2013.
Makeover valued at approx. $250.
Winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received at the studios of 21Alive/NBC33,, 3401 Butler Road, Fort Wayne, IN 46808 Winner will be notified via email and/or telephone. Winner's image may be used on-air on the networks of WPTA 21Alive, WISE NBC33 and online at www.INCnow.tv.
This contest is void where prohibited by law. Employees of Granite Broadcasting, Malara Broadcasting Group, contest sponsors, their respective parents, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible. The winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their offices, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
