Must be over 18 to enter. Winners will be chosen at random. You may enter once per day. Contest winner must reside within the following Indiana counties: Adams, Allen, Dekalb, Huntington, Jay, Noble, Steuben, Wabash, Wells, Whitley or the following Ohio counties: Paulding and Van Wert.
Prize Value: approx $100. Number of Winners: 7
Winner will be required to sign a release form and show a state or federal issued identification Winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received at the studios of 21Alive/NBC33, 3401 Butler Road, Fort Wayne, IN 46808 Winner will be notified via email and/or telephone. Submitted photos may be used on-air on the networks of 21Alive/NBC33 and/or online at www.INCnow.tv. This contest is void where prohibited by law. Employees of Granite Broadcasting, Malara Broadcasting Group, contest sponsors, their respective parents, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible. The winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their offices, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
