Curtis Smith Golf Challenge 2013 Rules:
Rules: NO PURCHASE NECESSARY
Entry deadline: noon September 15, 2013.
Must be over 18 to enter and play.
One two-person team will be chosen every week, beginning Monday, May 6th. Play to take place on Monday mornings at 9:00am (weather permitting) at Autumn Ridge Golf Club / 11420 Auburn Rd Fort Wayne, IN 46845. Rain dates will be attempted within the given week, but not guaranteed.
Prizes:
- $500 in golf merchandise, IF the two-person team wins match round play on 18 holes.
- A Lexus ES 350 IF one member of the two-person team scores a hole-in-one on the hole designated by Autumn Ridge Golf Club.
Winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received at the studios of Indiana's NewsCenter, 3401 Butler Road, Fort Wayne, IN 46808.
Winner will be notified via email and/or telephone. Winner's image may be used on-air on the networks of Indiana's NewsCenter and online at www.INCnow.tv.
This contest is void where prohibited by law. Employees of Granite Broadcasting, Malara Broadcasting Group, contest sponsors, their respective parents, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible. The winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their offices, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
